Bergen Care Management Program
Catholic Charities provides care-management services
for older adults in partnership with Bergen NJ EASE,
a program of the Bergen County Division of Senior Services. Our services
target frail, vulnerable, low-income adults, 60 years of age and older,
who are at risk for premature institutionalization.
